
COLORADO POST-HOSPITAL CARE
At Kaiser Permanente Colorado, your care doesn’t end when you’re discharged from the hospital. Even after you leave, the Kaiser Permanente Post-Acute Care Team (or PACT) is with you to help ensure a smooth transition and a comfortable recovery.
What we do
As you begin the recovery process, your care team will create an individual plan just for you. If you go home to continue your recovery, depending on the nature of your condition, PACT may send an advanced practice nurse to your home to address any complex needs you have. We also will pair you with a social worker to connect you with community resources, if needed. If you require more hands-on care, Kaiser Permanente Colorado partners with top-notch home health agencies and skilled nursing facilities so that you receive the services you need. PACT works closely with these partners to help provide seamless, quality care to get you back to your healthiest.
Recovering at home is always the first choice, but there are times when you might need more extensive care and therapy before you return home. We have several contracted partners throughout the Denver area to provide this care.
Who we are
We are a dynamic team of experts who are dedicated to your healing. We’re highly qualified, board-certified doctors, advanced practice nurses, registered nurses, and social workers who are with you from the time you enter the hospital for treatment to the time you go home and continue your recovery. Our team includes:
- Geriatric- and hospital-trained physicians rounding in our skilled nursing facilities
- Advanced practice nurses providing care in your home
- Licensed clinical social workers in all of our partner hospitals
- Care navigators to partner with you and help develop recovery plans
Connected care
PACT partners with your primary care provider and any other specialist involved in your treatment. Kaiser Permanente Colorado’s integrated care system allows us to give you the most appropriate care you need, when you need it. Your electronic health record allows us to collaborate with many specialists in different areas of medicine, which gives you the best chance to recover from a serious illness or injury, your way.
