A better model for treating heart failure
The number of Americans living with heart failure is projected to increase 46% by 2030.2 Heart failure occurs when the heart does not pump as well as it should, causing fluid to build up in the lungs and other parts of the body.
Through the Kaiser Permanente Integrated Care for Improved Heart Function Program, our members with heart failure are paired with a multidisciplinary team that creates personalized treatment plans, coordinates referrals, and uses high-tech tools to monitor them remotely. With this ongoing support, members are able to adjust to heart-healthy routines that help improve their heart function so they can live longer, healthier lives.
